Offering help
A direct and informal offer, literally 'I help you?'. Use with friends, family, or people your age.
The formal version of '¿Te ayudo?'. Use with older people, in professional settings, or with anyone you'd address as 'usted'.
A general and polite offer, meaning 'Can I help with anything?'. It's slightly less direct than '¿Le ayudo?'.
Literally 'At your order/service'. Very common in service contexts like shops and restaurants, especially in Colombia and other parts of Latin America.

`¿Le puedo ayudar?` or the more formal `¿En qué le puedo servir?` ('How can I serve you?') are very common in stores and restaurants.
`A la orden` is used constantly by vendors and shopkeepers, both as an offer of help and a general greeting. `¿En qué le puedo colaborar?` ('How can I collaborate/help?') is also common.
The `vos` form is used for informal address, so you'll hear `¿Necesitás ayuda?` instead of `¿Necesitas ayuda?`.
The informal plural `vosotros` is used, so a group would be asked `¿Os ayudo?` ('Can I help you all?'). `¿Necesitas algo?` ('Do you need anything?') is also a very frequent and slightly more casual offer.
Using the wrong level of formality. Saying `¿Necesitas ayuda?` (informal `tú`) to an older person or a store clerk can sound disrespectful. When in doubt, use the formal `¿Necesita ayuda?` (for `usted`).
Confusing `ayudar` (to help) with `asistir` (to attend). `Asistir` usually means 'to attend' an event. Saying `¿Puedo asistirle?` might be misunderstood as 'Can I attend you?'. Stick with `ayudar` for offering help.
Making a statement instead of asking a question. In Spanish, questions start with `¿` and end with `?`. `Necesita ayuda.` is a statement ('He/she needs help'). You must use a rising intonation and the correct punctuation to ask the question: `¿Necesita ayuda?`.
